Mandatory Online Check in 

All teams must check in online prior to the tournament through GotSport.  Deadline to check in is Friday, May 24th (10:00 PM EST). Successful online check-in confirmation emails will be sent by Wednesday prior to the tournament to the primary team contact.

Documentation Requirements (to be uploaded to GotSport)

  1. Certified/Validated Team roster  (USYSA, US Club)
    • REC TEAMS: 2 copies of completed Roster/Waiver and verification that your players are current Rec players  
  2. Guest Player Roster approved/validated by the State Association where player plays or Player Loan Form approved by both teams
  3. Copy of Players, Guest Players, and Coaches Cards
  4. Permission to travel form from your state association if the team is outside of Ohio. US Club teams do NOT need a travel permit. Teams must be in good standing with their Federation Organization. 
  5. Concussion Certificate from coach
  6. Lindsay Law Team Roster and Team Information Form

Online / GotSport Check-In Instructions

  1. SCAN or SAVE each of the required documents as an individual PDF. 
  2. Upload the required documents to your GotSport account.
    • Log in to your coach/manager account and click on the “Team Management” button.
    • Click on the name of the team you are working on. Open the “Registrations” tab.
    • Click on the Event that you are Working to Upload Documents. Open the “Registration” tab. Next click on the “Edit” button.
    • For any files that are being asked to be uploaded, you will be able to click on the “choose file” button and upload accordingly. If you have a challenge uploading a document, there is a good chance that the document is too large and will need to be decreased in size.
